ok here it is.
How do you address a woman you have never met? As in a introduction letter.
I heard that (or read it actually) you address her by her first and middle names. Is that true? And are the women of Ukraine and Russia named after their fathers. I had an address from a lady named Olga Dmitrenko, very cute, er well anyway her middle name is Seergeyvna, it sounds familliar to Sergey. I was just wondering about names and introductions.

Well, yes, she woud be Olga Sergeyevna--but this is very formal sounding and would probably be more appropriate for business correspondance, rather than a personal note. I would just call her Olga in a first letter.

We have no middle names first of all. Sergeyevna is a patronymic which means that her Father's name is Sergey.

Call her Olga, and that's it. If you will see some warm feelings in her next letter, u can call her Olenka or Olechka after that. It's like affectionate names for Olga.
